Create Your Membership Plans
Define your membership tiers, pricing, and billing frequency so patients can enroll as soon as you are ready.
Membership plans are the billing backbone of your DPC practice in DPC Pro. Each plan defines what patients pay, how often they are billed, and what is included. You need at least one active plan before you can enroll patients.
Most DPC practices start with two to four plan tiers segmented by age range or membership type (individual, couple, family). You can create additional plans and modify existing ones at any time. Changes to a plan do not affect patients already enrolled unless you explicitly migrate them.
Prerequisites
Section titled “Prerequisites”- Set up your practice profile: your practice must exist before you can create plans
- Payment processing connected (optional but recommended). Navigate to Billing → Payment Setup to connect your payment processing account. Plans can be created without this, but you cannot collect payments until payment processing is active.
Create a Plan
Section titled “Create a Plan”- Navigate to Billing → Membership Plans.
- Select Create Plan.
- Enter a Plan Name: something patients will recognize, such as “Adult Individual” or “Family Membership.”
- Add a Description that explains what is included. This description is visible to patients during enrollment.
- Select the Billing Interval:
- Monthly: patients are billed once per month
- Annual: patients are billed once per year
- Select Save Plan.
Your plan is now active and available for patient enrollment.
Setting Pricing
Section titled “Setting Pricing”DPC Pro supports tiered pricing based on membership type. Each plan can have separate prices for individuals, couples, and families, with both monthly and annual options.
Price Tiers
Section titled “Price Tiers”When creating or editing a plan, you can set prices for up to three membership types:

For each tier, you can set:
- Monthly Price: the amount billed each month
- Annual Price: the amount billed once per year (typically at a discount compared to 12 months of the monthly rate)
At minimum, set the Single Monthly Price. The other tiers are optional. Leave them blank if you do not offer couple or family pricing on this plan.
Additional Member Pricing
Section titled “Additional Member Pricing”For family plans, you can configure charges for members beyond the included count:
- Set the number of Included Adults (default: 1) and Included Children (default: 0).
- Set the Maximum Members allowed on the plan (default: 4).
- Enter the Additional Adult Price: the per-person monthly charge for each adult beyond the included count.
- Enter the Additional Child Price: the per-person monthly charge for each child beyond the included count.
Age Definitions
Section titled “Age Definitions”DPC Pro uses age thresholds to determine whether a family member is classified as an adult or a child:
- Adult Age: the age at which a member is considered an adult (default: 18)
- Child Age Max: the maximum age for the child rate (default: 17)
These thresholds are set per plan, so you can define different age cutoffs for different plans if needed.
Billing Frequency
Section titled “Billing Frequency”The billing interval you select determines how often patients are charged:

Contract Terms
Section titled “Contract Terms”Each plan also has contract settings that control the minimum commitment period:
- Contract Length: the number of months a patient commits to when they enroll (default: 12 months). Set to
0for month-to-month with no commitment. - Auto-Renew: when enabled (the default), memberships automatically renew at the end of each billing period. Patients receive a renewal reminder before the renewal date.
Plan Visibility and Enrollment
Section titled “Plan Visibility and Enrollment”Active vs. Inactive Plans
Section titled “Active vs. Inactive Plans”Each plan has an Active toggle that controls whether it is available for new enrollments:
- Active plans appear in the plan selection list when enrolling new patients.
- Inactive plans are hidden from new enrollment but continue to work for patients already enrolled on them.
To change a plan’s status:
- Navigate to Billing → Membership Plans.
- Select the plan you want to update.
- Toggle Active on or off.
- Select Save.
Archiving Plans
Section titled “Archiving Plans”If you no longer need a plan at all, you can archive it:
- Navigate to Billing → Membership Plans.
- Select the plan you want to archive.
- Select Archive Plan.
Archived plans are removed from the plan list and cannot be used for new enrollments. Patients already enrolled on an archived plan keep their membership until it ends or is changed.
Family and Couple Plans
Section titled “Family and Couple Plans”DPC Pro handles family memberships through a single plan that covers multiple members under one account.
How Family Memberships Work
Section titled “How Family Memberships Work”- One person is the account holder and is responsible for payment. See Account Holders vs. Patients for how this role works.
- Additional family members are added to the membership as covered members, each with a relationship type (spouse/partner, child, parent, or other dependent). For day-to-day family management, see Managing Families.
- Pricing adjusts automatically based on the number of adults and children, using the rates you configured on the plan.
Setting Up a Family-Friendly Plan
Section titled “Setting Up a Family-Friendly Plan”- Create a plan with a descriptive name like “Family Membership.”
- Set the Single, Couple, and Family price tiers.
- Set Included Adults and Included Children to define how many members are covered at the base price.
- Set Additional Adult Price and Additional Child Price for members beyond the included count.
- Set Maximum Members to cap the total family size.
Example: Family Plan Pricing

In this example, a family of two adults and two children pays $150/month. A third child adds $25/month, bringing the total to $175/month.
What About Employer Plans?
Section titled “What About Employer Plans?”Employer-sponsored memberships use the same plan structure. See Employer Group Plans for the full setup guide. Create a plan with employer-appropriate pricing and enroll the employer’s employees as you would any other patient. The account holder can be the employer or the individual employee, depending on your arrangement.
Plan Benefits and Limits
Section titled “Plan Benefits and Limits”You can configure what each plan includes beyond the price:
