Patient Timeline and History

The patient timeline shows a chronological view of all clinical and administrative events: visits, notes, messages, billing changes, and more.

When you open a patient’s record in DPC Pro, the timeline provides a complete history of their interactions with your practice. Each entry is timestamped and categorized, so you can quickly scan for recent visits, review past clinical notes, or check when a membership change occurred.

This page covers how to read and navigate the timeline, filter it by event type, and use it during patient encounters to review relevant history. The timeline is a key tool for clinicians preparing for a visit or responding to a patient message.

The timeline grows automatically as events occur. No manual logging required.

The patient timeline presents events in reverse chronological order, with the most recent activity at the top.

There are several ways to view a patient’s clinical history:

  1. From the patient record: navigate to Patients, open the patient’s record, and review the sections displayed on the detail page. The Medical History section, practice enrollments, and linked actions provide a full view.
  2. From the clinical notes list: navigate to Clinical Notes and filter by the patient’s name. This shows all visit notes in date order with the note type, visit date, chief complaint, author, and status.
  3. From the note editor sidebar: while writing a note, the right-side context panel shows previous notes, upcoming visits, active prescriptions, and membership status for the current patient.

Each clinical note entry in the timeline shows:

  • Patient name and MRN
  • Note type (SOAP Note, Progress Note, Phone Note, Telehealth Note, Procedure Note, or Consultation Note)
  • Visit date
  • Chief complaint (truncated in the list view)
  • Author: the clinician who wrote the note
  • Status badge: color-coded indicator showing Draft, In Progress, Completed, Signed, or Amended
  • Amendment indicator: if a signed note has been amended, an “(amended)” label appears next to the status

The list is paginated at 25 notes per page, with page navigation at the bottom.

The patient timeline captures events from multiple areas of DPC Pro:

Visit notes are the primary clinical events. Each note records a patient encounter with full SOAP documentation, vitals, diagnosis codes, and linked prescriptions. Notes are visible on the clinical notes list and can be opened for full detail.

Prescriptions linked to visit notes appear in the note detail. Each prescription shows:

  • Medication name and strength
  • Dosage form
  • Directions
  • Quantity, refills remaining, and status (Draft, Sent to Pharmacy, Filled, or Cancelled)

Active prescriptions also appear in the note editor’s right-side context panel, giving clinicians a quick view of current medications while documenting a visit.

Updates to a patient’s medical history (allergies, chronic conditions, current medications, and surgical history) are reflected in the patient record. These entries are visible from the patient detail page and in the note editor sidebar during visits.

Scheduled visits appear in the patient’s record and are linked to clinical notes when documentation is created. From a note, you can navigate to the linked visit to see scheduling details including the visit type, date, time, and provider.

The note editor’s right-side context panel displays the patient’s current membership status and plan name, providing billing context during clinical documentation.

The clinical notes list offers several filters to narrow what you see:

  1. Search: type a patient name or chief complaint keyword in the search field. Results update as you type.
  2. Status: select from All Statuses, Draft, In Progress, Completed, Signed, or Amended to show only notes in a specific stage.
  3. Note Type: filter by SOAP Note, Progress Note, Phone Note, Telehealth Note, Procedure Note, or Consultation Note.
  4. Date Range: set a From and/or To date to limit results to a specific period.

Filters can be combined. For example, you can search for a patient’s name, filter to signed SOAP notes, and limit to the past six months, all at once.

To see all clinical activity for a single patient:

  • From the patient record, select View Clinical Notes in the actions panel. This opens the notes list pre-filtered to that patient.
  • From the notes list, search by the patient’s name.

When filtered by patient, the list header displays the patient’s name and avatar with a “Viewing all notes for this patient” label and a back arrow to return to the full list.

The note editor is designed to provide timeline context while you write, reducing the need to navigate away.

The left sidebar of the note editor displays:

  • Patient demographics: name, age, gender, and MRN
  • Allergies: displayed prominently in a highlighted panel. If no allergies are recorded, “NKDA” appears.
  • Current medications: up to five entries from the patient’s medical history
  • Chronic conditions: displayed as labeled tags
  • Surgical history: listed if recorded
  • Primary provider: the patient’s assigned provider at your practice

The right sidebar of the note editor (visible on larger screens) displays:

  • Linked visit: the visit that triggered this note, with type, date, and time
  • Upcoming visits: the next three scheduled visits for this patient
  • Previous notes: the five most recent notes for this patient, each linking to the note detail
  • Active prescriptions: up to five current prescriptions with medication name, strength, and dosage form
  • Patient info: allergies, membership status and plan, and date of last visit

While writing a note, you can open any previous note from the right sidebar:

  1. In the Previous Notes section of the right sidebar, select a note.
  2. The note detail opens in a new view showing the full SOAP content, vitals, diagnosis codes, and any amendments.
  3. Use your browser’s back button or the Back to Notes link to return to the note list, or navigate directly back to the note you were editing.

If the current note is linked to a scheduled visit, the linked visit details appear in the right sidebar showing the visit type, date, time, and provider. You can reference this information while documenting without leaving the note editor.
